About Emerge Surgical - Mandurah

Emerge Surgical, formerly known as Perth Bariatric Centre was founded by Dr Gordon Padovan following his return to Australia in 2005. His goal was to build a weight loss surgery practice that would provide a premium level of care to patients who are contemplating having Laparoscopic Gastric Bypass, Lap Sleeve or Lap Band surgery. Consulting Regularly in Mandurah and Perth (our main rooms) as well as Albany and Port Hedland, Dr Padovan strives to provide quality care and service. Emerge Surgical has been set up as a multi-disciplinary bariatric weight loss centre dedicated to achieving the best possible results. We have grown such that we now operate on hundreds of patients each year, and manage the long-term care of thousands of men and women who have undergone Bariatric Surgery procedures. Dr Padovan also looks after post surgery issues (excess skin removal procedures including tummy tuck and arm lifts), and performs a large amount of revisional surgery (removal of band, band to sleeve, etc.) and of recent, offers the SIPS procedure.

n adjustable gastric banding surgery (also known as lap band surgery) a hollow band made of special material is placed around the stomach near its upper end, creating a small pouch and a narrow passage into the larger remainder of the stomach. The lap band is then inflated with a salt solution. It can be tightened or loosened over time to change the size of the passage by increasing or decreasing the amount of salt solution.

Gastric Bypass (3 hours)

When we do the Gastric Bypass surgery, Roux en-Y, we want to make a very small pouch out of the upper stomach, to restrict the amount of food which can be eaten. That pouch is separated from the rest of the stomach, which is bypassed, by creating a new pathway into the intestines. This pathway is called a “Roux en-Y” (named after the French surgeon, Dr. Roux, who first described this reconstruction in the 1800’s). The bowel is cut, and reconstructed in a Y configuration, so that two parts of the GI tract can feed into one.

Whether we do the gastric bypass surgery through an incision, or laparoscopically, the basic methods, anatomy, and the results are the same, although the instruments differ. Here’s how we do the gastric bypass surgical procedure laparoscopically

Gastric Sleeve (2 hours)

 

Why choose the Laparoscopic Sleeve Gastrectomy?
  1. Your quality of eating with the gastric sleeve is superior to the band or bypass. For example, patients with the sleeve can eat any food once the staple line has healed and in small volumes.*
  2. With a gastric sleeve the amount of weight reduction is in the region of 60-70% of excess weight over the first 1-2 years with a modest regain long term!*
  3. Gastric sleeve surgery is a good option for FIFO workers and/or people living in remote areas because it is a “set and forget” operation which requires less post op follow up or nutritional supplements.*
  4. There is no malabsorption of nutrients.*
  5. There are no long term risks of slippage or erosion which are associated with the band.*
  6. Operative risks are slightly higher than the adjustable gastric band but lower than the gastric bypass.* The operative risks decrease with your surgeon’s experience. Dr Padovan has performed over 400 sleeve gastrectomies.
  7. With gastric sleeve surgery the staple line leak is the most serious complication occurring in 1-2% of the patients.* Of note, Dr. Padovan’s leak rate is currently 0.2% despite the fact he performs a lot of complicated revisional surgery.
  8. Laparoscopic Sleeve Gastrectomy is a good revisional surgery option for patients who have the adjustable gastric band and have regained weight or have experienced complications.* This can be done as a single stage or a two stage procedure depending upon the amount of scar tissue encountered intraoperatively in the area where the band was.
  9. If there is weight regain with the gastric sleeve, a gastric bypass can be performed, often laparoscopically as well.*

Biography:
Dr. Padovan is a bariatric (Obesity) surgeon and weight loss surgeon with a vast experience in Gastric Bands, Sleeve Gastrectomy and Laparoscopic Gastric Bypass. After completing his surgical training in Perth and obtaining FRACS in 1999, Dr Padovan spent a further five years subspecialty training in the USA. This included working at the University of Pittsburgh Medical Center, the leading Minimally Invasive Surgery Centre in the United States.
