The successful treatment of lines, wrinkles and furrows depends on their site, severity and extent.

There are a number of injectable, wrinkle-filling agents (fillers) available which are classified as:

• Semi-permanent (temporary)
• Permanent
• Implants which are permanent but removable

Ideally, a temporary wrinkle filler should not cause allergies, not move from its original placement, be safe, be injectable through a very fine needle to minimise discomfort, be undetectable once in place and be long lasting and be of minimal cost. At present no fillers satisfy all these criteria completely.

Permanent injectable fillers have been used for many years particularly for the lower part of the face. It is currently considered by most medical practitioners including ourselves that permanent injectable fillers should not be used to treat facial lines and wrinkles because of possible long-term complications. Therefore, the Dublin Cosmetic Surgery Centre does not offer any permanent injectable fillers.

Permanent implants have been in use for many years and many are now available for augmenting different parts of the face e.g. cheeks, chin, lips. They are made of inert materials to minimise tissue reaction. The commonest material to be used is silicone.

A permanent implant differs from a filler in that it comes prepared as a solid material which can be carved/modified in size and shape and which will not react with human tissue. In addition once placed under the skin it can be readily removed. Thus an implant will be sited in a pocket under the skin whereas a filler once injected will dissipate (spread) into the subcutaneous tissue.

Fat Filler

The transfer of fat has been extensively employed for many years to fill out a variety of contour defects on the surface of the body. Fat transfer (Lipotransfer) involves transplanting fat cells (containing the fat) and is regarded as a type of graft. In their new position, the fat cells continue as before by absorbing and storing fat.

Excessive Sweating Treatment

Excessive sweating or hyperhydrosis occurs in about 1% of the population and is thought to be of genetic origin. It is caused by over activity of one type of sweat gland (the eccrine gland). These glands are found throughout the body but are concentrated on the palms of the hands, soles of the feet and in the armpits.

It is possible to inject the skin of these areas and block the action of the nerves that supply the sweat glands thereby preventing them from producing sweat.

Lip Augmentation

The ageing process affects the lips which results in the opening of the mouth gradually moving downwards. When a young girl smiles she generally shows the fronts of her top teeth. In middle age, she is likely to show the gap between the upper and lower teeth. When she is elderly she will often show the fronts of her lower teeth. This process is due to the lengthening and thinning of the upper lip. Often the height of the pink or vermilion part of the lip reduces as well.

Younger patients may wish to have more prominent and pouting lips. Synthetic injectable fillers, fat and implants can be used to achieve this. The technique of using temporary fillers and implants to produce this look has been named the “Paris Lip”. The aim of a “Paris Lip” procedure is to increase the bulk of the upper and/or lower lips by enhancing the borders of the lips and giving more definition.

Brow Lift

The Dublin Cosmetic Surgery Centre has found from many years of experience that the surgical treatment of brow lifting has proved disappointing.

There are 2 surgical procedures of brow lift:

1. The Traditional Brow Lift

A long horizontal incision across the scalp inside the hairline is required. This has proved unsatisfactory to a large number of female patients who dislike the transverse scar and find it difficult to style their hair.

2. Endoscopic Brow Lift

In this procedure, the scalp is raised from its attachment to the bone and stretched backwards using keyhole surgery. In our experience this procedure proved disappointing as results were far too short lasting.

In the current climate where patients are looking for less expensive and non-invasive solutions we have had excellent results using the hibernation treatment. With a course of injections, the muscles of the forehead which lower (depress) the brow are de-activated. The muscles which elevate the forehead are unaffected and are free to lift the brow.

Eyelid surgery

Ageing around the eyes is associated with the formation of excess skin folds and eye bags. Eye bags are especially seen in the inner or medial parts of the upper eyelid and along the length of the lower eyelid.

The aim of eyelid surgery (blepharoplasty), otherwise known as eye bag removal is to reduce the excess eyelid skin and to flatten/remove the eye bags. Eyelid surgery (blepharoplasty) can be carried out separately on the upper or lower eyelids or both upper and lower eyelids together.

Chest reduction is an increasingly popular operation in males which removes/reduces enlarged male breasts, commonly referred to as "man boobs" or “moobs”.

There are 3 reasons why the male breast enlarges:

1. If there is an abnormal increase in glandular (breast) tissue known as Gynaecomastia.
2. If there is an increased deposition of fatty tissue known as Pseudogynaecomastia
3. If there is an increase of both glandular and fatty tissue.

The most common reason for enlarged breasts in males is Pseudogynaecomastia.

During adolescence it is common for the male breast to enlarge and later to decrease to normal size before the age of 21.

Gynaecomastia in the late adolescent and adult male may be associated with hormone disorders, hormone producing tumours, liver disease and other rare abnormalities. In addition, various drugs, if taken for long enough can give rise to this embarrassing condition. Usually though, no underlying cause is found.

Inverted Nipple Surgery

In the majority of cases, inverted nipples occur as a result of a congenital defect caused by an underdevelopment of the milk ducts, which retract the nipples as the breast develops. Inverted nipples can also be acquired after certain breast diseases e.g. mastitis, breast cancer, following breast surgery or after childbirth.

Inverted nipples can be a major cause of embarrassment to the sufferer but are readily amenable to correction by surgery.

The purpose of inverted nipple correction is to pull out or evert the hidden nipples so that they protrude from the centre of the areola.

Protruding ears (“Bat Ears”) is an inherited condition and can cause significant embarrassment especially if a person is constantly teased. In all people the ears are not completely symmetrical and it is usual for one ear to be further out than the other.

There are two ways in which the ears can protrude more than usual:

1. The inner part of the ear flap (concha) might be unusually long or large.
2. The angle between the concha and the outer part of the ear flap (helix) might not be properly formed so the helix extends sideways rather than going backwards (the usual position).

Often both of the above conditions exist in severe cases of protruding ears.

The purpose of an ear reshape operation (otoplasty) is to reduce the distance between the side of the head and the protruding part of the ear to about 15mm (1/2”).

Ear reshape (otoplasty) can also (a) reduce the size and projection of the concha if it is too big and/or (b) establish the normal angle between the concha and the helix.

It is not usually possible to reduce the size of very large ears that have a normal shape due to the complex shape of the ear.

Skin resurfacing involves removing the superficial layer of the skin and allowing the raw skin beneath to heal itself. This stimulates the production of new collagen, which rejuvenates the skin, resulting in a smoother skin surface. During this process minor blemishes and scars are also removed.

Dermabrasion or “sanding” of the skin is a technique used in selected cases and sites to remove irregularities in the skin surface. Dermabrasion is performed by using a rotating wire brush or diamond wheel to plane down the irregular area. The smooth, even surface that results, produces an improved appearance.
